Wording Help Needed

My boss has asked me to word an waiver/disclaimer form for a customer to sign. It is at the customers request. Basically, when we unload the machine there are these lifting eyes (holes for cable to go in) used to get the machine off the truck. The form basically just has to say that we are not liable if the customer is injured while using these lifting eyes. Here's the point form notes he gave me to go from:

These are Quickmill’s swivel lifting eyes that we use to lift the machine onto…
We are not liable if the customer is injured while using these lifting eyes.
I suck at wording...HELP!!!
